Membership Application Form

An Odd Fellow lodge is a good place to be. And, it's lots of fun. You'll like meeting good people, making new friends, enjoying the pleasures of new experiences. It's where the action is in most communities. Sports, socials, dances, and parties are all part of the active life of Odd Fellows today.


Apply for Membership

When the lodge has received your application, they will appoint an interviewing committee of three members. These members will call on you, both to ask questions and to answer any questions you may have concerning the lodge. One purpose of the interviewing committee is to give a candidate at least three familiar faces in the lodge. At the next meeting, the interviewing committee will report back to the lodge. The lodge will ballot to accept you as a member. If you are accepted, the lodge will then send you a letter announcing your election to membership and the date and time for initiation.

Requirements and Joining Procedure
Contact a lodge or lodge member:
  • Use the online Membership Request Form
  • Or contact your local I.O.O.F. Lodge.

    • Complete an application:
  • Received from a member or download one below.

    • Your completed application & initiation fees:
  • Will be forwarded to the Lodge.

    • The Noble Grand (President) of the Lodge assigns members to investigate:
  • An investigation of the overall character of applicant takes place.

    • An Interview of the Applicant:
  • Application will be interviewed by the Investigating Committee.

    • The Interviewing Committee reports to members:
  • Reports & recommendations will be given during a meeting regarding the Applicant.


    • Members vote on the Applicant

    The Applicant is advised of the voting results.

    The Applicant receives an initiation notice.

    The Applicant receives Initiation in the Lodge.
